‘We know far more about our medicines than our critics assume’

By Dr A. Shanthakumar

Once, after reading a highly critical article on homoeopathy, a person asked me out of genuine curiosity: “How can homoeopaths practise medicine without knowing anything about their medicines?”

His question reflected a widespread misconception. He had been led to believe that homoeopaths simply administer sweet pills blindly because, as critics often claim, “there is nothing in homoeopathic medicines”.

Such criticism frequently overlooks an important fact: homoeopathy possesses an extensive body of literature concerning its medicinal substances, their preparation, their observed effects, their clinical applications, and their therapeutic relationships.

Whether one accepts or rejects the theoretical interpretations of homoeopathy, it is incorrect to assume that homoeopaths practise without a detailed medicinal literature.

What Do We Know About Our Medicines?

The following categories of information are extensively documented throughout homoeopathic literature and have historically formed the basis of homoeopathic education and clinical practice.

1. Source, Identity, Properties, and Preparation

We study the source of each medicine, whether botanical, mineral, chemical, zoological, or biological.

Classical and modern homoeopathic pharmacy describes the identity of medicinal sources, their physical and chemical characteristics, biological properties where relevant, known constituents or active principles, toxicological information, collection of medicinal materials, and methods of preparation and preservation.

For plant-derived medicines, even factors such as the part of the plant used and the appropriate stage or season of collection may be specified.

Modern analytical investigations have also examined homoeopathic preparations using spectroscopy, materials science, interfacial chemistry, and nanoscience. These investigations remain areas of continuing scientific discussion and research. As in every branch of medicine, complete knowledge has not yet been achieved, and further rigorous investigation is necessary.

2. Observed Effects on Healthy Provers

Homoeopathy has developed an extensive record of symptoms historically attributed to medicinal substances during systematic observations on healthy individuals, traditionally known as ‘drug provings’.

These observations form an important part of the Homoeopathic Materia Medica.

Our literature includes source Materia Medica, proving records, clinical Materia Medica, comparative Materia Medica, and therapeutic works.

Thus, a homoeopath does not merely learn the name of a medicine. The student studies an extensive characteristic profile traditionally associated with that medicine.

3. Sphere of Action and Tissue or Location Affinity

Homoeopathic literature describes what is traditionally called the “sphere of action” of individual medicines.

The descriptions may include affinities for particular organs, tissues, anatomical regions, and sides of the body.

The literature sometimes records highly specific locations, including the mucocutaneous junctions, mastoid region, intercostal spaces, insertion of the deltoid muscle, wrist joints, ankle joints, and other defined anatomical sites.

Whether all such historical observations are ultimately confirmed by modern experimental methods is a legitimate subject for research. Nevertheless, these detailed descriptions undeniably exist in homoeopathic literature and are actively used in traditional remedy differentiation.

4. Duration of Action

Classical homoeopathic literature contains descriptions of the presumed duration of action of individual medicines.

For different remedies, the period of therapeutic activity has traditionally been described in terms of hours, days, or longer periods.

These observations have historically influenced repetition of the dose and clinical follow-up.

5. Age, Constitution, and Individual Susceptibility

Our Materia Medica describes particular medicines as having greater relevance to certain age groups, constitutional patterns, and characteristic types of patients.

Infants, children, adolescents, pregnant women, elderly persons, debilitated patients, and individuals with particular patterns of susceptibility are separately considered in remedy differentiation.

The purpose is not simply to treat a disease name but to examine the individual expression of illness.

6. Seasonal and Climatic Relationships

Homoeopathic literature records medicines traditionally associated with complaints arising or worsening during particular seasons and climatic conditions.

Heat, cold, damp weather, dry weather, storms, seasonal transitions, and exposure to particular environmental conditions are considered during case analysis.

Certain remedies are also described as less appropriate under particular circumstances.

7. Effects of Food and Beverages

Homoeopathic literature contains detailed observations regarding foods, beverages, stimulants, and other substances believed to influence medicinal action.

Some substances have traditionally been regarded as interfering with or antidoting the action of particular remedies.

Whether every classical interaction remains valid requires critical scientific evaluation, but the existence of such detailed pharmacological and clinical observations cannot simply be ignored.

8. Time and Circumstances of Maximum Symptom Expression

An important feature of Homoeopathic Materia Medica is the documentation of modalities.

Symptoms may be described as worsening or improving at a particular time of day, during movement or rest, from heat or cold, from pressure, after eating, before menstruation, during sleep, on waking, or under numerous other circumstances.

These modalities are used to differentiate medicines that might otherwise appear very similar.

9. Remedies Following Unsuccessful Prescriptions

Homoeopathic literature discusses medicines that may be considered when a previously selected remedy has failed to produce the expected clinical response.

Such decisions are not supposed to be arbitrary. The case must be reassessed and the subsequent prescription selected according to the changed or remaining symptom picture.

10. Complementary Remedies

Certain medicines are described as complementary to others.

A complementary remedy is traditionally understood as one that may continue or complete a therapeutic process initiated by a previous medicine when the evolving case corresponds to the second remedy.

These relationships have been recorded through generations of clinical observation.

11. Antidotal Relationships

Our literature also records medicines traditionally considered capable of counteracting unwanted effects attributed to another remedy.

Remedy relationships were systematically studied by early homoeopathic physicians.

Boenninghausen’s Therapeutic Pocket Book, for example, contains extensive consideration of remedy relationships. Such information has historically assisted practitioners in analyzing follow-up prescriptions and sequences of remedies.

12. Sequences of Remedies

Homoeopathic literature describes certain remedies as following one another well in particular clinical situations.

Thus, the practitioner may study not only an individual medicine but also its relationship to medicines prescribed before or after it.

This concept of remedy sequence represents an important component of classical homoeopathic case management.

13. Potency and Dosage

The same homoeopathic medicinal source may be prepared in different potencies.

Homoeopathic literature contains extensive discussions concerning potency selection, dose, repetition, susceptibility, vitality, pathological condition, and response to previous prescriptions.

There have always been differences of opinion among homoeopathic schools regarding these questions. Nevertheless, it is incorrect to claim that homoeopathy has no literature or guidelines concerning potency and dosage.

14. Complaints Attributed to Foods, Beverages, and Drugs

Homoeopathic repertories contain extensive sections concerning complaints arising after particular foods, beverages, medicinal substances, and environmental exposures.

The category traditionally called ‘ailments from’ is remarkably detailed.

The practitioner may investigate symptoms following alcohol, coffee, particular foods, pharmaceutical drugs, exposure to cold, heat, or numerous other circumstances.

15. Ailments Following Physical and Emotional Events

Homoeopathic literature gives considerable attention to illnesses or symptom patterns following identifiable events.

These include grief, fright, anger, disappointment, emotional shock, physical injury, overexertion, sleep deprivation, changes in lifestyle, and exposure to environmental agents.

The temporal relationship between an event and the development of symptoms is considered important in individual case analysis.

16. Potency Considerations in Special Clinical Situations

Classical and clinical literature contains recommendations concerning potency and repetition in children, highly sensitive individuals, debilitated patients, persons traditionally described as having low vitality, and patients with advanced illness.

Such recommendations differ among authors and schools and should not be confused with experimentally established pharmacological laws.

Nevertheless, they demonstrate that homoeopathic prescribing has historically involved detailed consideration of patient susceptibility and clinical condition.

17. Assessment of Direction of Clinical Change

Homoeopathic philosophy contains detailed discussions concerning the interpretation of changes following treatment.

Practitioners are taught to observe whether the patient is improving generally, whether important functions are recovering, whether symptoms are changing in a favourable or unfavourable direction, and whether the clinical picture suggests progression of disease.

Classical authors attempted to formulate principles for distinguishing improvement from suppression, deterioration, and poor prognosis.

Modern medicine rightly demands that such interpretations be correlated with objective clinical examination, laboratory investigation, imaging, and validated outcome measures whenever appropriate.

An Enormous but Scattered Body of Knowledge

The information described above is not contained in a single book. It is scattered throughout the Organon of Medicine, Hahnemann’s lesser writings, homoeopathic philosophical works, proving records, source books, Materia Medica, repertories, pharmacy texts, therapeutic manuals, clinical literature, and research publications.

Additional observations have appeared in homoeopathic journals, theses, conference papers, clinical reports, and writings of both historical pioneers and contemporary physicians.

Therefore, the real problem is not that homoeopaths possess no information about their medicines.

The real problem is that this enormous body of information remains fragmented, historically expressed, and insufficiently integrated with modern chemistry, molecular biology, pharmacology, toxicology, structural biology, and contemporary scientific methodology.

The Next Responsibility of Homoeopathy

Our responsibility today is not merely to repeat what our old books have written.

We must critically examine it.

We must classify it.

We must digitize it.

We must test reproducibility.

We must distinguish reliable observations from historical assumptions.

We must correlate characteristic medicinal profiles with toxicology, molecular structures, active constituents, biological pathways, pathological molecules, and measurable clinical outcomes.

Where a claim is wrong, it must be corrected.

Where an observation is reproducible, it must be scientifically investigated.

Where a mechanism remains unknown, we must have the intellectual honesty to call it a hypothesis rather than an established fact.

This is particularly important in the emerging framework of Molecular Imprint Therapeutics (MIT), in which we propose investigating whether highly diluted, succussed preparations may contain persistent supramolecular recognition structures capable of functioning as artificial binding sites for complementary pathological molecular targets.

MIT must not be protected from scientific criticism. It must be experimentally tested.

Homoeopathy will not become scientific merely by repeatedly declaring that it is scientific.

It will advance when its enormous historical medicinal database is subjected to chemistry, molecular biology, spectroscopy, binding studies, structural analysis, reproducibility testing, and properly designed clinical research.

We know far more about our medicines than many critics assume.

But knowing what our literature says is only the beginning.

Our task now is to determine, by scientific investigation, what is reproducible, what is measurable, what is molecularly explainable, and what must be revised.

That is the path from traditional Homoeopathic Materia Medica to modern Molecular Imprint Therapeutics.
